FN3109H NVR - cannot add Foscam C1 Camera

I have a FN3109H NVR with (3) Foscam cameras.  The first 2 Foscam cameras were able to connect to the FN3109H without issue, but the last camera, a C1, will not add.  I am 100% confident that I have the right IP address, port, and credentials for the C1.  I say this because have deleted and re-added the C1 several times during troubleshooting to my existing monitoring software (Blue Iris), and I have also logged into the camera itself several times during troubleshooting.  Whenever I add the camera in the setup page of the FN3109H, it does not return any errors or warnings.  It simply acts like it added it, but no video appears.   As an additional torubleshooting step, I have updated the firmware of the FN3109h & the C1 camera to the latest release.   The C1 has no issues with Blue Iris or the 3rd party app on my phone, but it simply refuses to work with the Foscam NVR.  I am stumped.

the third camera may have experienced a malfunction.
